Now, use that fabulous information to educate those kids!  Choose 1 of the below writing products:


*I AM POEM (title will be "I am_____(disease)_______  remember, the "I" is you talking as the disease. Use attached template as a guide (you can modify however you need to and add more to make sure you co
ver the information.)

*ABC POEM:  use the information you generated to complete a Poem using words and phrases for the letters of the alphabet.  You can be creative with your spelling for certain letters.

*Journal  (for example:  "My Life before, during, and after ____(disease)__________"  View this as writing a diary as a person's habits and life living with a certain disease.


*Fable:  (for example:  once upon a time....pick characters/animals to use in your story that have bad habits and describe their way through life using your information on a certain disease).


*Cartoon 



Information what needs to be included in each final product: (see rubric on the bottom of page)

Prevention:  what to do to prevent the illness from occurring
Risk Factors:  
what puts people at risk for getting the disease?
Warning Signs:  
how do they know they are developing the disease?
Treatments/cost:  
what now...when they have it?
